3. If you have forgotten your User Name, please click on the ‘Forgot your user name?’ link to receive your User Name.
Note: Your @PremierHealth.com email address is the ONLY email that will retrieve your User Name.
4. If you have forgotten your password, please click on the ‘Forgot your password?’ link to receive a Temporary Password/Access Code by email. You will need to enter this Temporary Access Code twice before creating a new password.
